![]() ![]() Lynch has been involved with DTSD sports for over a decade. The Athletic Trainers coordinate care and appointments with Dr. Silvis clears all concussions sustained by any DTSD athlete and is available for sports injury consultations. He lives in Hershey with his wife, Christine, and their two sons, Nicholas and Benjamin. Silvis enjoys hiking, mountain biking, running, and photography. He has special interests in the care of endurance athletes, exercise, and concussion. ![]() He also serves as team physician for the Hershey Bears Hockey Club. ![]() Silvis is currently an Associate Professor in the Departments of Family and Community Medicine and Orthopaedics and Rehabilitation and medical director of primary care sports medicine. Silvis practiced wilderness medicine in rural Wyoming before returning to Wake Forest where he completed a fellowship in primary care sports medicine. He then completed a Family Medicine Residency at Wake Forest University in Winston-Salem, NC. He is the Director of Primary Care Sports Medicine Fellowship, graduated from Pennsylvania State College of Medicine in 2002. Silvis has been a Sports Medicine Physician for DTSD since 2010. Students’ applications are entered into HACC’s online RCS (Residency Certification System), by the district office once the district office has all the completed paperwork. If the student is unable to apply for a Certificate of Residency in person, a parent or spouse may do so by providing the above documentation as required.Ī completed application form (from the district office) and the verified tax payment can be dropped off at the District Administration Office, located at 30 East Granada Avenue, Hershey from 8:00 AM to 4:00 PM Monday through Friday (summer hours will vary). Applicants must have a HACC ID# before applying for a Certificate of Residency and must present photo ID. ALL students other than the current Derry Township high school graduating class must present documentation confirming they are up to date with theirSchool Occupation Tax for Derry Township. To qualify for a certificate, students must have resided in Derry Township for six (6) consecutive months as of July 1. Derry Township School District is a sponsoring district of the Harrisburg Area Community College (HACC). ![]()
